As you are stitching use your bottom hand for a guide. I poke my middle finger each time my needle comes through to the back and use my hand to make sure the fabric on the back is not bunched up...no puckers.
Now go to the bottom picture in this post.
Then tug the thread until the knot disappears behind the quilt top. Trim off the thread where it came up.

Take your needle down about 1/8 inch away from where the thread was coming up, stay on the stitching line so it does not look funny. Be careful to only go through the top layer of fabric and carry the needle point away about 1/2 inch or so before poking it back out the top fabric.
When you are nearly out of thread or are done with an area and need to end the thread then make a double knot about 1/4 inch away from the fabric.
